FORM Face + Body Opens FORM Campus in Midtown Toronto
Five clinics under one roof, and an Orientation Day on October 14 to welcome the city in
TORONTO, Sept. 22, 2026 /CNW/ — FORM Face + Body today announced the opening of the FORM Campus at 90 Eglinton Avenue East, a 23,000-square-foot destination bringing plastic surgery, aesthetic medicine, hair restoration, men’s health, women’s health, and longevity care together under one roof.
To mark the opening, FORM will host Orientation Day on Wednesday, October 14, a campus-themed open house where guests can tour the clinics and meet the physicians who lead them.
FORM has reimagined the traditional aesthetics, plastic surgery and wellness model. Rather than a single practice, the FORM Campus is organized as five specialized clinics, each led by physicians with deep academic and surgical training, working side by side in midtown Toronto.
From surgical interventions to ongoing health optimization, the campus is designed to support individuals across their full spectrum of care.

- FORM Face + Body. Cosmetic surgery of the face, body and breast, with non-surgical injectables, skin and laser treatments.
- FORM Hair. Hair preservation, hair transplant and hair restoration.
- FORM Men’s Health. A physician-led men’s health program directed by Dr. Noah Stern, combining subspecialty diagnostics with an integrated model of care.
- FORM Women’s Health by Élevé. Pelvic health, incontinence, sexual wellness and hormone optimization, led by Dr. Alaina Garbens.
- FORM Life+. A dual purpose clinic focused on healthy aging and longevity care combined with non-surgical aesthetics led by Drs. Cory Goldberg and Mike Brandt, offered through individual treatments, comprehensive assessments and annual memberships.

ORIENTATION DAY
We invite guests to arrive at the new FORM Campus as new students. You will receive a lanyard and student ID, a campus map and an orientation kit, then explore the campus at your own pace, join small group sessions and meet the faculty. Guests who complete the tour will receive a certificate.
